2 workplace fatalities at an Illinois factory

According to police, two workers died at a liquid feed manufacturer in Pekin on June 24. The incident occurred shortly after 12 p.m. at Agridyne LLC located on South Second Street.

The fatal accident happened as the plant's employees were doing regular maintenance and cleaning work on several parked train cars when one of the workers slipped and fell into one of the cars and was overtaken by deadly fumes inside of it. Following the first accident, another employee died after jumping inside the train car to try to save the worker, authorities reported. Another employee who was at the scene notified law enforcement officials for help.

According to preliminary investigations, the two workers allegedly died from carbon monoxide poisoning, which rescue and fire personnel stated were at dangerously high levels inside the cars at the time of the incident. However, it was reported that investigators were still working to determine all the factors involved in the workers' deaths.

Sometimes, workplace accidents can be fatal.
